37-years old woman (asymptomatic except fatigue) with JAK2 wild type thrombocythemia: platelets 1205 x10/9/L, Hb 12.5 g/dl, leukocytes 18 x10/9/L, slight increase of LDH, spleen not palpable but 13 cm on scan (normal value <12cm) as the presenting features of primary megakaryocytic granulocytic myeloproliferation (PMGM , dry tap; RF-2) diagnosed in 2013 as CALR mutated PMGM according to 2006-2018 ECMP criteria [34,47-50] and meeting the 2008/2016 WHO criteria for primary myelofibrosis (PMF) [24,29].
